On our home page there is a "Schedule An Appointment" widget that will bring you to "OnPatient" our patient portal. You may also call to schedule (302)-271-4669 or email us at jo@choicehealthde.com.
Access your patient portal, select your appointment and click cancel appointment. Appointments should be cancelled at least 24 hours in advance. If you cannot access your patient portal call or email us. (302)-271-4669 or jo@choicehealthde.com
Your initial appointment is a comprehensive assessment and needs to be in person, please allow 45-60 mins. All follow up appointments can either be done in-person or via Telehealth, please allow 15-30 mins.
After scheduling your appointment via "OnPatient" you will have the option to check-in; from there you will have access to the video call.
You will be able to select your contact preferences during your initial visit.
If you need a medication refilled, send a message to your provider through your patient portal. Your provider will e-fax a prescription to the pharmacy, unless a follow-up appointment is necessary for the medication refill.
All results will be discussed either over the phone or in person depending on scheduling. After which, they will be readily available on your patient portal.
